چکیده

The management of cancer involves surgery, radiotherapy and chemotherapy. Development of chemoresistance is a persistent problem during the chemotherapy treatment. Cytotoxic drugs that selectively, but not exclusively, target actively proliferating cells include such diverse groups as DNA-alkylating agents, anti-metabolites, intercalating agents and mitotic inhibitors. Resistance constitutes a lack of response to drug-induced tumour growth inhibition; it may be inherent in a subpopulation of heterogeneous cancer cells or be acquired as a cellular response to drug exposure. Principle mechanisms may include altered membrane transport involving the p-glycoprotein product of the multidrug resistance (MDR) gene as well as other associated proteins, altered target enzyme, decreased drug activation, increased drug degradation due to altered expression of drug metabolising enzymes, drug inactivation due to conjugation with increased glutathione, subcellular redistribution, drug interaction, enhanced DNA repair and failure to apoptosis as a result of mutated cell cycle proteins such as p53. Attempts to overcome resistance involves the use of combination drug therapy using different classes of drugs with minimally overlapping toxicities to allow maximal dosages, necessary for bone marrow recovery. Adjuvant therapy with p-glycoprotein inhibitors and in specific instances, the use of growth factor and protein kinase C inhibitors are newer experimental approaches that may also prove effective in delaying onset of resistance. Gene knockout using antisense molecules may be effective way of blocking drug resistance.

Newest Targets for Anticonvulsant Agents: An Overview

Epilepsy is a neurological condition characterized by recurrent seizures influencing about 1% of the worldwide population. Despite much progress in understanding the pathogenesis of epilepsy, the molecular basis of human epilepsy still remains unclear. Common approaches for pharmacotherapy of epilepsy are still directed towards controlling the symptoms and suppression of seizures. Clinical use ...

Cancer cells acquire resistance to anticancer drugs: an update.

The efficacy of cancer chemotherapy is often affected by the emergence of resistant can-cer cells. While biochemical and pharmaco-logical mechanisms have been proposed to ex-plain chemo-resistance, the genes involved in this process have not been fully identified. We previously used genomic DNA microarrays and quantitative RT-PCR to identify the genes associated with resistance to chemotherapeu...

Mechanisms of cancer drug resistance.

The design of cancer chemotherapy has become increasingly sophisticated, yet there is no cancer treatment that is 100% effective against disseminated cancer. Resistance to treatment with anticancer drugs results from a variety of factors including individual variations in patients and somatic cell genetic differences in tumors, even those from the same tissue of origin. Frequently resistance is...

Intracellular drug sequestration events associated with the emergence of multidrug resistance: a mechanistic review.

The acquisition of multi-drug resistance (MDR) in cancer cells subjected to anticancer agents remains a formidable obstacle to successful therapeutic outcomes in cancer patients. As the name implies, the resistance phenotype (MDR) is not typically limited to the drug initially used to eradicate cancer but is often transferred to structurally unrelated chemotherapeutic agents. The mechanisms und...

Leishmaniasis: drugs in the clinic, resistance and new developments.

The control of Leishmania infections relies primarily on chemotherapy. The arsenal of drugs available for treating Leishmania infections is limited and includes pentavalent antimonials, pentamidine, amphotericin B, miltefosine, fluconazole and few other drugs at various stages of their development process. In this review, we will discuss the latest results regarding resistance mechanisms to dru...

Antifungal drug resistance of yeasts in dermatology: mechanisms, epidemiology and clinical impact

Superficial dermatophytoses are among the most common infectious disease. The most commonly used antifungal categories reazoles, polyenes and echinocandins. Due to the limited number of available antifungal drugs, toxicity and the emergence of resistant (intrinsic or acquired) strains, antifungal strategy needs to be developed. Recently the researchers try to find alternative antifungal agents....
